Understanding your receipt and remittance form

Your unique CFCA ID, which is shown on both your receipt and remittance form above your name and address, links you directly to each child or aging person you sponsor, and allows us to instantly identify your relationships.

Receipt (top portion)

Keep this top portion for your records.

Your receipt provides detailed information about your sponsorship and donations. For example, your year-to-date contributions are noted, along with a listing of any donations made for Birthday, Christmas, Project or General funds, or any other special donations.

If you sponsor more than one child or aging friend or give to several funds, you receive one receipt detailing your contributions instead of multiple receipts.

Remittance form (bottom portion)

Return this bottom tear-off portion with your next contribution.

Your remittance form contains your CFCA ID and other important information. The reverse side of the form gives you the option of signing up for our Automatic Payment Plan. With automatic payment, you can make your contributions by credit card or automatic bank withdrawl.

Payment of your sponsorship by automatic bank withdrawl reduces our costs, and allows us to send those savings to CFCA projects. Your payment may be scheduled for 5th or 20th of the month and can be canceled at any time.

CFCA ID: Your CFCA ID links you directly to each child or aging person you sponsor. With your CFCA ID, the remittance form has all the needed information for accurate and efficient processing of your contributions.

Sponsorship: Fill in the amount of your sponsorship contribution. If you sponsor more than one child or aging friend, be sure to include the total amount you are contributing for all your sponsorships.

Special donations: Fill in the amount of any special donations to the Birthday, Christmas, Project or General funds on the appropriate line.

Total enclosed: Total the amount of your sponsorship contribution and any special donations.
